- fully complete and sign IMM5444E, when need input current date and if you failed to input "now", input a date same with the date you will sign the doc(for example 18/02/21).
- prepare passport's photocopy.
- prepare 2 W5cm*H7cm photos (into a small envelope) with info written at the back of the photos:
- Name and date of birth
- Date, name and address of photography studio
- photocopy of birth certificate, if under 18 years old.
- report cards, transcripts, and attendance records, if under 18 years old.
- pay fee and print receipt.
- make sure everything is ready, and mail them together with the checklist(imm5644e) to:
